目的：探究口服血府逐瘀丸联合玻璃体腔注射阿柏西普治疗视网膜静脉阻塞(RVO)的疗效。

方法：前瞻性研究。选取2021年1月至2024年3月期间我院收治的RVO患者80例80眼，随机分为对照组40例采用玻璃体腔注射阿柏西普治疗和观察组40例采用口服血府逐瘀丸联合玻璃体腔注射阿柏西普治疗。比较两组患者治疗RVO的疗效、中医证候积分、视网膜微循环参数、黄斑水肿(ME)以及不良反应。

结果：所有患者均完成随访。观察组总有效率(95%)高于对照组(80%； χ²=4.114，P=0.043)； 治疗3 mo后，两组患者中医证候积分、黄斑中心凹无血管区(FAZ)面积、FAZ周长、黄斑中心视网膜厚度以及新生血管渗漏面积均较治疗前降低，视网膜浅层毛细血管丛(SCP)血流密度以及视网膜深层毛细血管丛(DCP)血流密度均较治疗前上升，且观察组优于对照组(均P<0.05)； 两组患者治疗期间不良反应比较无差异(P>0.05)。

结论：口服血府逐瘀丸联合玻璃体腔注射阿柏西普治疗RVO可改善视网膜微循环，提高视力，恢复眼部血液循环，改善出血情况，促进ME吸收，提高临床疗效。

METHODS: A total of 80 patients(80 eyes)with RVO admitted to our hospital from January 2021 to March 2024 were prospectively selected. According to the treatment method, they were divided into a control group of 40 patients treated with intravitreal injection of aflibercept, and an observation group of 40 patients treated with oral Xuefu Zhuyu Pill combined with intravitreal injection of aflibercept. The efficacy, TCM syndrome score, retinal microcirculation parameters, macular edema(ME), and adverse reactions were compared between the two groups of patients in the treatment of RVO.

RESULTS: All patients have completed follow-up. The clinical effective rate of the observation group after treatment was 95%, obviously higher than that of the control group(80%; χ²=4.114, P=0.043). After treatment for 3 mo, the traditional Chinese medicine syndrome scores, foveal avascular area(FAZ)area, FAZ circumference, macular central retinal thickness, and neovascularization leakage area of both groups decreased, the overall blood flow density of the superficial capillary plexus(SCP)and deep capillary plexuses(DCP)increased, and the observation group showed better results than the control group(all P<0.05). There was no statistically significant difference in the occurrence of adverse reactions between the two groups during the treatment period(P>0.05).

CONCLUSION: Oral administration of Xuefu Zhuyu Pill combined with intravitreal injection of aflibercept in the treatment of RVO can improve retinal microcirculation, enhance vision, restore ocular blood circulation, improve bleeding, promote ME absorption, and improve clinical efficacy.
