目的:探讨中药有效成分梓醇是否通过靶向甲基转移酶样蛋白3METTL3-m6A-硫氧还蛋白相互作用蛋白(TXNIP)轴,抑制NLRP3炎症小体的活化,从而减轻糖尿病视网膜血管内皮细胞损伤。

方法:体内实验部分,采用链脲佐菌素诱导的糖尿病小鼠模型(每组20只),通过视网膜电图评估视网膜功能,通过伊文思蓝渗漏实验评估血管完整性。体外实验部分,将人视网膜血管内皮细胞(HRVEC)暴露于高糖环境(30 mmol/L),并分别给予梓醇或METTL3抑制剂STM2457处理。随后检测以下指标:采用斑点印迹法检测整体m6A水平及METTL3的表达;采用免疫印迹法检测NLRP3炎症小体组分及TXNIP的表达;采用DCFH-DA探针检测氧化应激水平。此外,通过分子对接与分子动力学模拟分析梓醇与METTL3NLRP3TXNIP及白细胞介素-1βIL-1β)的结合情况。

结果:梓醇治疗可改善糖尿病小鼠的视网膜电图振幅(包括a波、b波及振荡电位),减轻视网膜血管渗漏(P<0.05),同时下调视网膜组织中血管内皮生长因子(VEGF)、NLRP3IL-1βIL-18的蛋白水平。在高糖刺激的HRVEC中,梓醇可抑制NLRP3炎症小体激活,减少活性氧释放,下调METTL3表达及整体m6A甲基化水平(P<0.05),并逆转高糖诱导的TXNIP上调。METTL3抑制剂STM2457可模拟梓醇的上述全部保护效应。分子对接与分子动力学模拟结果证实,梓醇能与METTL3NLRP3TXNIPIL-1β稳定结合。

结论:梓醇通过抑制NLRP3炎症小体减轻糖尿病视网膜血管内皮损伤,其机制至少部分归因于下调METTL3依赖性TXNIPm6A RNA甲基化。

目的:探讨不同眼轴长度高度近视合并黄斑前膜(ERM)患者的临床特征及手术治疗效果。

方法:回顾性分析20125月至202111月于本院接受玻璃体切割手术治疗的高度近视ERM患者。根据眼轴长度(AXL)分为26–<28 mm组、28–<30 mm组及≥30 mm组,比较各组患者术前临床特征及术后视功能和黄斑解剖结构变化。

结果:共纳入73例患者(男性27例,女性46例)76只眼,平均年龄(60.5±9.2)岁。三组患者术前最佳矫正视力(BCVA)比较无统计学意义差异(P=0.498)。本研究发现,随着AXL增加,外层黄斑劈裂、内外层黄斑劈裂并存、椭圆体带(ellipsoid zoneEZ)中断及中心凹视网膜脱离的发生率均明显升高(均P<0.05)。术后仅26–<28 mmBCVA较术前显著改善。总体上,77.6%的患眼术后视力提高。而术后视力未改善的患眼中,58.8%来自26–<28 mm组。术后1眼继发黄斑裂孔(1.32%)。多元线性回归分析显示,术前BCVAP<0.001)及视网膜前增殖(EP)(P=0.010)与术后最终BCVA相关。

结论OK镜可缩小双眼间屈光参差;DIMS镜对近视性屈光参差患儿双眼近视控制效果相似。对于高度近视患者,OK镜疗效优于DIMS镜片;当屈光度低于-3.00D时,DIMS镜片对AL轴延长的延缓作用优于OK镜片。

原发性开角型青光眼(POAG)是一种慢性致盲性眼病,以视网膜神经节细胞(RGCs)进行性凋亡为主要特征。随着全球人口老龄化加剧,其发病率及致盲率持续攀升。非病理性高度近视(HM)已被证实是POAG的独立危险因素,且二者在视神经损伤早期均存在相似的筛板结构改变。本综述归纳POAGHM状态下筛板的共性及差异性病理改变,旨在阐明筛板在视神经损伤发生与进展过程中的核心作用。明确二者病理改变的异同点,有助于从组织结构层面实现高危人群的早期筛查,并制定针对性干预方案。
